The medical institutions within the STARCLINIC system (including all STARCLINIC located
throughout Taiwan, hereinafter referred to as “the Clinic”) operate on the basis of
mutual trust between our clients and the Clinic. By using this website and related
services, or by registering as a member of the Clinic, you agree to the collection,
handling, and use of your personal data in accordance with the principles set forth in
this Privacy Policy.
The principles of the Clinic’s Privacy Policy are as
follows:

The Clinic will collect personal data that is relevant and necessary for its business operations, including your name, telephone number, date of birth, national identification number, email address, mailing address, and other personal information required for specific purposes consistent with the Clinic’s scope of services.

Our Data Collection Notice
I. To handle communications between you and the Clinic and to provide information about appropriate products and services.
II. For the purposes described in Paragraph 1 above, as well as for matters within the scope of the Clinic’s registered business activities, Articles of Incorporation, or other purposes permitted by law (including but not limited to marketing conducted independently or jointly with others, consumer protection, consumer/customer management and services, e-commerce services, account administration, contractual or quasi-contractual relationships and other legal matters, surveys, statistics, research analysis, information and database management, and the collection, handling, and use of personal data by non-government agencies as required by law).
III. To provide personal data to the recipients described in Paragraph 3 below for purposes consistent with their registered business activities, Articles of Incorporation, or as otherwise permitted by law. The Clinic may collect, handle, use, and internationally transmit your relevant personal data (as detailed in Paragraph 2). In accordance with the provisions of the Personal Data Protection Act, we hereby inform you of the following matters. Please read them carefully.
- Specific purposes for collecting personal data: In accordance with the specific purposes and categories of personal data under the Personal Data Protection Act promulgated by the Ministry of Justice, the specific purposes for which your personal data is collected are detailed in Appendix I.
- Categories of personal data collected: The categories of personal data collected are based on the data actually obtained through the Clinic’s interactions and business dealings with you. Depending on the needs of different business activities, the Clinic may collect your personal data in the following five categories listed in the specific purposes and categories of personal data under the Personal Data Protection Act promulgated by the Ministry of Justice:
- − Identification information (C001–C002), such as name, telephone number, bank account number, or credit card number
- − Personal characteristics (C011–C013), such as gender and date of birth
- − Social circumstances (C031–C041), such as residential address, property information, and residency documentation
- − Financial transactions (C093), such as payment and receipt amounts, credit limits, payment methods, and transaction records
- − Other information (C131–C132), such as email addresses and other information that cannot be classified under the aforementioned categories
IV. Period, area, recipients, and methods of personal data use:
- 1. Period: The retention period corresponding to the specific purpose for which the personal data was collected; the retention period prescribed by the applicable laws and regulations or contractual agreements governing the preservation of such data (e.g., under the Business Entity Accounting Act); or the retention period established by the Clinic (whichever is longer than the period required by law).
- 2. Area: The Republic of China, including Taiwan, Penghu, Kinmen, and Matsu.
- 3. Recipients: The Clinic; organizations affiliated or cooperating with the Clinic (such as 新星顧問有限公司, Starry Biotechnology Co., Ltd., and StellarCell Biomedicine Inc.); outsourced service providers; third parties having cooperative, agency, or commissioned relationships with the Clinic (including but not limited to organizations engaged to conduct market research); recipients of internationally transferred personal data not restricted by the competent central authority; the Clinic’s joint marketing and promotional partners; other organizations having business or transactional relationships with the Clinic; competent authorities and other regulatory agencies having jurisdiction over the Clinic; judicial authorities; and other government agencies.
- 4. Methods: By automated devices or other non-automated methods.
V. Pursuant to Article 3 of the Personal Data Protection Act, you may exercise the following rights with respect to your personal data held by the Clinic:
- 1. Except as otherwise stipulated by Article 10 of the Personal Data Protection Act, you may inquire about, request access to, or request copies of your personal data. The Clinic may charge necessary costs as permitted by law.
- 2. You may request supplementation or correction of your personal data, provided that you appropriately explain the reasons and supporting facts as required by law.
- 3. You may request that the Clinic cease collecting, handling, or using your personal data, or request the deletion thereof. However, this shall not apply where the Clinic must retain or use the data to carry out its business operations as required by law.

VI. The impact of declining to provide personal data:
You may choose whether to provide the relevant personal data. However, if you decline to
provide the relevant data, the Clinic may be unable to conduct the necessary review and
processing operations, and therefore may be unable to provide related services. In such
cases, the Clinic reserves the right to decline your application for or provision of
such services.
VII. You agree that the Clinic reserves the right to revise this notice, and that following any revision, the Clinic may notify you of the key amendments and the designated webpage through verbal communication, written notice, telephone, text message, email, facsimile, electronic documents, announcements at branch locations, website postings, or any other method reasonably capable of informing you or enabling you to become aware of such revisions (including but not limited to providing a link to a webpage containing the full text of this notice). At that time, please review the contents of the designated webpage carefully.
VIII. If any provisions regarding the collection, handling, use, or international transfer of personal data contained in agreements or documents previously signed by you or the Clinic conflict with this notice, this notice shall prevail.

In accordance with the specific purposes and categories of personal data under the Personal Data Protection Act promulgated by the Ministry of Justice, the specific purposes for which the Clinic collects your personal data are as follows:
